Before You Start

Before you begin applying as a Global Mission Worker, please ensure you have all of the required files ready. You will be asked to attach and upload each later on in the form. Here are the files we require:

A current photograph of yourself / yourselves.
Letter of Commendation – If you do not have a copy of your Letter of Commendation, MSC may have a copy on file that we can supply you with. Please email Conrad to check.
Worker Agency Agreement – Please sign and have your elders sign the worker agency agreement then scan the signed document, which you will upload later in this form.
Informed Consent Form – If you will be serving outside Canada, please fill out and upload your informed consent form.
CCI Security Course – If you will be serving outside Canada, MSC will reimburse the cost of this course to your account after receiving proof of completion. Please save a screenshot of the results page when you are finished. The cost is $35 USD which will be reimbursed in CAD on your account once results are submitted. If your spouse takes the course with you, a submitted certificate will represent both of you.
CCI and Informed Consent Exemption If you live and serve in Canada, you are exempt from the CCI Security course and Informed Consent forms - please download and attach this exemption when asked for these items.
Police Check w/ Vulnerable Sector Screening – Please upload your Police Check w/ Vulnerable Sector Screening. This must be less than 3 years old. Every regional police force handles checks differently. If yours would like a letter from MSC requesting the check (or incentivizes checks for charities), Conrad can supply you with one upon request.
Plan to Protect Certificate – A prepaid link to take MSC's customized Plan to Protect course was emailed to you. If you don’t see an email from Plan to Protect in your inbox, please check your spam folder, inbox tabs, and email search. If you don’t find it in any of those places, email Conrad. Please save your Certificate after completing the course. The course must be completed within 30 days of when you begin or it will expire and you will have to restart at your own expense.

If you are registering with your commended spouse, you also need the following files for them:

Training and Preparation

CCI Security Course*

Risk Management Profile

Please enter a number value from 1 (low) to 10 (high) to indicate your assessment of the threat level for each of the items listed below. The number represents the threat that each item poses to you in your specific situation and location. When possible, try to think locally more than nationally, the numbers should be for the particular places where you will be serving.

Think of average conditions in Canada being (1), and higher numbers representing an increased threat. For example, extremely poor roads equal a higher threat (8) and very good medical treatment would equal a lower threat (2).

It is expected that you have researched credible online sources such as: irinnews.org, travel.gc.ca, travel.state.gov and sources in the field prior to completing this form.
